Six Collaborative Objectives

Early Detection

Objective 1 - All GPs to use an agreed protocol to diagnose, treat and care for people with dementia.

Communication

Objective 2 - Everyone with dementia to receive appropriate, timely information. All carers of people with dementia to receive appropriate, timely information.

Person-centred Care

Objective 3 - Everyone with dementia to have their views taken into account when planning care and receive a copy of their care plan.  All carers of people with dementia to have their views taken into account when planning care and receive a copy of their care plan.

Objective 4 - Everyone with dementia to be enabled to pursue activities that are meaningful and appropriate to their interests.

Objective 5 - Arrangements exist to enable people with dementia and their carers to influence planning, delivery and evaluation of services.

Care Co-ordination

Objective 6 - All professionals involved in care of people with dementia to have a copy of the care plan.
